To receive your prize you must complete the following steps:
- KEEP your original receipt.
- DOWNLOAD and PRINT the declaration and release form (see the link below). You will also receive an email with the declaration form
- COMPLETE the release form and ensure you and a witness sign it.
- RETURN the completed form within 10 days to us by email or mail (see contact details on the release form).
- If you are declared a winner, you will be notified via email, and your $100 Walmart Gift Card will be mailed to the address you entered with.

Prize Declaration and Release Form to confirm that you are eligible to claim the above prize. Once this happens, you will be notified that you have won the prize, and then please allow up to 8 weeks for your prize to arrive by mail.
If you fail to submit the completed Declaration and Release within the time allotted, or you fail to correctly answer the skill-testing question, the Sponsor or Contest’s administrator shall conclude that the selected entrant has forfeited the Prize, which may then be awarded to an alternate eligible entrant at the Sponsor’s discretion.
Also - you have been automatically entered (assuming your entry and receipt are valid according to the rules and regulations) into the sweepstakes draws for the (3) Grand Prizes of $5,000 Walmart Gift Cards. This draw will take place on April 7, 2026 as per the rules and regulations. Each of the Grand Prize winners will be notified after that date.
