You must complete the following steps:

1. KEEP your original receipt.

2. DOWNLOAD and PRINT the declaration and release form (see the link below). You will also receive an email with the declaration form

3. COMPLETE the release form and ensure you and a witness sign it.

4. RETURN the completed form within 10 days to us by email or mail (see contact details on the release form).

5. If you are declared a winner, you will be notified via email, and your $100 Walmart Gift Card will be mailed to the address you entered with.

If you fail to submit the completed Declaration and Release within the time allotted, or you fail to correctly answer the skill-testing question, the Sponsor or Contest’s administrator shall conclude that the selected entrant has forfeited the Prize, which may then be awarded to an alternate eligible entrant at the Sponsor’s discretion.
